Meg NielsenOutreach Development Manager
The Outreach Development Manager seeks to expand awareness of, and communicate with, the greater Madison community, inclusive of churches, non-profit organizations and for-profit organizations, and to build and strengthen community among the residents. The Outreach Development Manager develops fundraising events, coordinates volunteer members of churches, student groups, businesses and civic organizations and raises awareness of TCM through media contacts, Facebook posts, web updates and speaking engagements. Together with the Chaplain and Community Parish Nurse, the Outreach Development Manager arranges and participates in on-site activities that vary from social gatherings to educational and spiritual opportunities.

Community Parish Nurse
Our Parish Registered Nurse provides confidential community health nursing care that promotes physical, mental, emotional, environmental and spiritual wellbeing to any resident of the Triangle Community. Services are offered free of charge to the residents. This important health ministry supports individual and community wellness and safety, enabling residents to live independently in better health and to avoid unnecessary hospitalizations.

Nursing services may include:

•  health evaluations to assist residents to identify personal health
    care priorities,

•  individual and group education on health issues and trends,

•  crisis intervention and support during times of illness or grief,

•  referrals and advocacy for health care and social service resources,

•  direction and support to successfully navigate the health care system
and maintain a health care home.

Our nurse also partners with Edgewood College to serve as facilitator and mentor for baccalaureate nursing students. This relationship provides a unique and most welcome opportunity for Triangle residents to share their stories and health care experiences in educating future nurses.

TCM’s Community Parish Nurse Program is offered in partnership with St. Mary’s Hospital and is further supported by a grant from United Way of Dane County.
